Please take a few minutes to read our guidelines for teleconsultation using PlatoConnect.

If you are keen to proceed with the scheduling of your teleconsultation, please reply with the following details in the following format:

    • Preferred Date:

    • Other person to participate in teleconsultation: YES/NO

      If yes, do provide the following information:

      • Relationship to patient

Please give us 1 to 2 working days to confirm your appointment with you. Do note that your preferred appointment date and time is subject to the availability of our doctors.

GUIDELINES FOR TELECONSULTATION USING PLATOCONNECT

Kindly note of the following appointment scheduling for teleconsultation during this Circuit Breaker period is subject to the doctor’s availability.

Mon to Fri: 8.00 am to 4.00 pm (Clinic early closure at 4.30 pm)

2) Involvement of other person to participate in teleconsultation

If you are scheduling on behalf of a patient or you are a patient and would like another person to participate in the tele-consultation, do provide us with this information:

    1. Name of person
    2. Relationship to patient
    3. Contact number
    4. Email address

Kindly note that a maximum of 2 people including the patient is allowed in each teleconsultation and we will require the patient to give us their consent to allow the above-named person to participate.

You will receive a consent form by email and you are required to submit this to us complete with your digital signature. We must receiver this signed consent form before we can hold the teleconsultation.

3) About PlatoConnect

PlatoConnect is a secure, private, peer-to-peer service. Your teleconsultation session will not be recorded.

4) Confirmation of teleconsult appointment and preparations

Upon the confirmation of your appointment, you will receive an invite link by email.

If you are joining the call from your computer, you may connect in via Google Chrome or Firefox browser.

If you are joining the call from your mobile device, you will be required to download the Plato Connect app from the Apple App Store or Google Play in order to connect. Please do test the function of your camera and audio device before your scheduled appointment, especially if you are connecting from your computer, so as not to cause unnecessary delay or disruption to your appointment.

5) Preferred mode of payment

You will receive a “Credit Card Authorisation” form to fill out to give us authorisation to charge your card only for the visit date stated. We will charge your card on the day of consult prior to your appointment. If you wish to pay by another payment method, please contact our clinic staff for the options available.

6) Joining the call

Upon joining the call, your will be requested to show your photo identification (NRIC, Work Pass, Passport) for verification purposes. This will ensure that only authorised persons are participating in the tele-consultation.

7) After the call

Our clinic staff will be in touch with you after the end of the call. They will go through with you any follow up plans, medications to be dispensed and outstanding payment, if required.
